When our baby, Pao-pao, got confined..we needed to prepare the requirement for Philhealth to be able to get a claim to lessen our expenses for the hospital. And it was really a great help for us…it maybe not the whole amount but it was a great less.
Here’s are some tips and list of requirements.
REQUIREMENTS:

CF1 FORM
You can get this from the hospital or download it at www.philhealth.gov.ph. Fill your information together with the patient’s information then give it to your employer to be filled-up.

CERTIFICATE OF CONTRIBUTION
This is a certificate of your contribution to PhilHealth from your employer. It should state at least 6 months of contribution. Stating your participating amount, date of payment and OR number.

MDR (Member Data Record)
This your record from Philhealth together with your beneficiaries. This is a proof that your are registered at Philhealth. You can get this at any Philhealth center near you.

PROOF OF RELATION
If the patient is not registered with your Philhealth (shown in your MDR), you need to provide this requirement.
If the patient is your spouse, you need a Marriage Certificate. And if the patient is your child, parent, or sibling, you need their Birth Certificate.
TIPS:

COMPLETE AND COMPILE AHEAD OF TIME
Prepare all the documents needed prior to confinement or as early as the day of confinement. It is easier to get the claim when it is included in your bill than reimbursing it after the confinement.

ASK YOUR DOCTOR
Ask you doctor, if he/she is accepting Philhealth. So that they will be informed that they have to deduct Philhealth from their charge.

KNOW YOUR BENEFITS

SUBMIT REQUIREMENTS
Submit all the documents and requirements prior to the check-out at the hospital; so that if the hospital has an additional requirements or things to be changed, you will still have time to take care of it. And with this manner, your check-out will be processed easily.

Hi Floro,
I don’t have a copy for this. Our copy was submitted to the hospital as a requirement. This is a letter stating your contributions backed-up with a photocopy of the receipt paid to Philhealth by the company.

Good day mam,
Am a member of Philhealth for less than three years, But my contribution stop and continue depending upon on my job situation.
For example, 10 months of job therefore ten months of contribution, then, when vacation occur my contribution will stop, and when job resume contribution also resume.
In this year, i contribute for the months of January-March then July-September. therefore theres a gap of three months
In my case now, can i still used my Philhealth if in case of emergency?

Yes you can but when you claim, you have to update your contribution. One of the requirements in claiming is to show your latest 3-6 months payment receipt.

I went to the emergency of St. Luke’s Hospital where I was given several blood tests, citiscan and ultrasound…I was charged with a bill of 25k pesos for which I paid through my credit card..I am a senior citizen but did not know about the benefits, can I have my 25k reimbursed by applying for the philhealth benefit? Hi my mother was admitted in the hospital.
I’m Ofw . My problems is my mother and father is not legally marriage. In short I’m illegitimate child but I bring the family name of my father. When my mother has admitted in the hospital . There is problems arises because she still used her singleness family name of my mother because they are not legally marriage Either in church or civil wedding. May I ask what are the requirements needed to submit just to claim my Mother as my beneficiary? Is it allowed to reimburse the confinement bill of the hospital after discharge ? My one year old baby was confine in a private hospital and he did not yet enrolled to my PhilHealth and i was afraid to continuously increase our bill. I am lock of time to fix those papers cause have a long time process and days. What can i do? Can the birthcertificate of my child’s enough for proof to use my Philhealth as fast as his father also?
